Case Records of the Urgent Care 05: Black and Blue

A 25 year old African American pt stopped by today for a doctor’s note. This is one of the most common reasons for visits here. She stated she got into an “altercation” four days prior. Pt remained vague on the details of this altercation, or who it was with. She did not seek medical attention at the time. She stated that she wants time off because her injuries still hurt. Pt is downcast and on the verge of tears. She says the injuries consist of bruises. She denies any loss of consciousness, dizziness, change in vision or hearing, weakness, numbness.

Examination shows healing bruises on both upper arms. Pt is an expert at makeup, but shows me a photo on her phone of a black eye. No other injuries were evident.

I WAS 100% SURE OF THE DIAGNOSIS AS SOON AS I WALKED IN THE DOOR. WHAT IS YOUR GUESS?

Speaking of psychopaths, today brings news of the indictment of another racist cop who decided to punish an act of disrespect from a black person with execution.

Officer Raymond Tensing’s execution of Samuel DuBose follows the typical model.

1. There is a routine police encounter over a minor infraction. In this case, driving with one of the two required plates not installed. Other offenses punished by summary execution include selling looseys, running away from the cops on foot, and shoplifting.

2. The unarmed black person fails to show the expected submission to the officer, and either talks back (Garner, Bland) or else attempts to flee (Scott, DuBose).

3. The officer then deploys lethal force.

There is usually no step between 2 and 3.

A black man disrespecting a certain type of cop is a capital offense.

When I was in college, I remember the cops were clearing out a drunken rager one time. My white roommate said something along the lines of, don’t you have anything better to do? The cop walked up to him and gave him this psychotic look of death… but then walked away. That was scary enough, but if my roommate had been black? I had no doubt that he would have immediately taken a baton to the solar plexus, followed by a hard takedown with or without a tasing or pepper spraying, followed by his being left to rot in jail for the weekend, followed by his being charged for resisting arrest, disorderly conduct, and whatever else they could dream up. Or if my hypothetical black roommate had attempted to flee, he would have been just another statistic alongside Eric Garner, Walter Scott, Oscar Grant, and now Samuel DuBose.

I really don’t have much else to add that hasn’t been said a thousand times over already with each one of these executions. Just, it never seems to change, does it?

Most internet angry mobs are unjust. Small crimes and misdemeanors get blown all out of proportion as they get passed around Twitter and Facebook, with each new addition to the mob more angry than the last. Virtual torches and pitchforks are wielded as they come for their largely female victims… but really, did Justine Sacco deserve to have her life ruined over one drunken racist tweet? Did a small-town pizza shop run by rednecks deserve to be shut down over a relatively mild homophobic remark?

So it is a great relief to see that today’s internet angry mob is fully justified in their wrath. They are after a sadist and monster who might not even be punishable by the law, as long as he stays out of Zimbabwe — Walter Palmer, the vile dentist who decided to show how manly he was by paying $55,000 to kill a lion, with a crossbow and rifle, while assisted by locals, after luring it out of a nature preserve. So not only is the online mob correct, but they also may be the only force able to bring any punishment at all to this psychopath.

His clinic’s Facebook pages was taken down after it was flooded with insults. His Yelp page got the same treatment. He is Twitter’s biggest villain. Comment sections make him roughly as popular as ISIS. All the media sites hoist his image as the villain du jour.

His dental practice now faces financial ruin. He will most likely face investigation by the state dental board, with possible sanctions, which would never have happened otherwise. More immediately, his clients will run and not walk to competing practices to get away from this cruel villain — would you trust this man with your health and your life while under laughing gas?

So, let’s hear it for the angry mob for getting it right for once. Unlike Sacco, their target deserves every last bit of what they are dishing. Every #CecilTheLion tweet and every mention on Facebook, on Kik, on Tumblr, and on WordPress helps further ostracize and isolate this atrocious individual. And not only for his just deserts, but also for our safety — the chilling lack of any resemblance of empathy or humanity that led Walter Palmer down this road makes him far more likely than the average citizen to start hunting the most dangerous game of all… if he hasn’t already done so in his past. (Fortunately, he appears too much of a coward to actually become a serial killer, even if such urges frequently cross the space where his soul was supposed to be.)

It’s not often the internet mob is justified in ruining a man’s life, but in this case, there is no upper limit to the invective which this individual deserves. So, go nuts, my lovelies… just don’t break any laws (well, no felonies at least) while making Walter Palmer about as popular as Electronic Arts, Hitler, and anal warts to internet users!

update: here is Kevin Drum with more discussion on the internet mob mentality, and how it usually dispenses disproportionate or wholly inappropriate punishment. This is true, but this also makes the wholly justified angry-mob response to Walter Palmer all the more fascinating. I expect many psychology doctoral dissertations to touch on this news event in the future.

Is the guy in the middle a Bush voter? Must be!

Earlier, I attributed Trump’s temporary ascent in the polls to the Moron Demographic — those voters who also are the base for Sarah Palin, Rob Ford and Marion Berry.

While morons make up part of Trump’s poll bump, I underestimated just how powerful Trump’s racist diatribe against Mexicans was for nativist and white supremacist conservatives. That rant, and his refusal to apologize, has called forth such individuals to his cause while shaking the GOP establishment to its core.

These voters are currently blowing up the right-o-sphere by calling mainstream Republicans “cuckservatives” (and we’ll get to this confusing insult in a moment) while declaring that Trump, for all his flaws, is the only guy willing to stand up to what they see as the self-loathing GOP establishment, or “GOPe” in their shorthand. Intentionally or not, Trump has become the standard-bearer for white-nationalist Republicans, who have not had a national candidate since George Wallace — and thus, Trump is exposing the GOP’s darkest secret: its base of overtly racist voters.

This is why GOP chairman Reince Priebus wants Trump out of the debates. Not because he would turn off viewers — on the contrary, I expect the Aug. 6 debates to have far and away the highest ratings for a primary debate of all time if Roger Ailes allows him on stage. No, it is because Trump illustrates for the world media just where many Republican voters are really coming from.

These men (for the vast majority of them are white men) have always pulled the correct lever for the GOP every four years. They may not like the Bushes and Romneys they are called to vote for, but they sure hate the Dems a lot more. These white nationalists were brought into the GOP fold by Nixon’s Southern Strategy, which capitalized on the “Dixiecrat” rebellion of the 1960s, personified by Wallace. These voters abandoned the Democratic party in search of a new home, and Nixon’s operatives heard them. And the South has remained deep-red ever since.

But while white nationalists’ loathing of liberals, minorities and feminism make them more rock-solid Republican voters than the hosts of Fox and Friends, they have always felt ignored and neglected by their own party. For good reason: major Republican candidates have learned not to overtly embrace these guys, lest they go out like Trent Lott. Yes, they will accept their campaign contributions, and yes, they may try an occasional dog whistle; but conventional GOP pols like Walker, Bush and Rubio would rather be caught french-kissing Hillary than be photographed anywhere near a Confederate flag.

Which brings us to the white nationalists’ latest insult to such conventional GOP pols: the “cuckservative.” What on earth does this mean?

Another day, another hateful shooting spree

Just another evening in America.

Last night, a loser drifter attempted to beat Dylann Roof and Mohammod Youssef Abdulazeez’s “scores” at a Louisiana theater. Fortunately he failed to kill more than two people, perhaps due to the heroism and quick thinking of two wounded women. (What a sad statement it is if we’re happy that “only” two die during one of these things!)

We don’t know the motive yet — was he a James Holmes copycat, or was he an Elliot Rodger-type woman-hating obsessive? Evidence to the latter: the two confirmed dead victims were women, in addition to the women from above. In addition, hate killers are far more common than motiveless psycho killers these days. Of course, this is not conclusive — hate killers tend to leave behind their pathetic “manifestos,” so we will see if one surfaces. Psychos like Holmes kill indiscriminately and often don’t leave manifestos. But if John Russell Houser was indeed targeting women, then he joins Rodgers, Roof and Abdulazeez in the swelling ranks of hateful, evil, loser men.

The only abnormality is his age. But while hate-inspired spree killers tend to be young, this is not a hard and fast rule. For instance, George Sodini, 48, consumed by hatred of women, killed three before killing himself.

Either way, there is an ongoing crisis with men in this society that shows no signs of going away. I spin my wheels on this, but I get no closer to understanding where their hate and evil actually originates from. The specific targets of their hate — blacks, women, infidels, family members, or just random people — vary, but the vile disease does not. And nobody seems to be any closer to understanding it.

Taylor Swift is getting piled on by Nicki Minaj, Kim Kardashian, and now Katy Perry in the Twitter dust-up that will someday be called “The Day Perez Hilton Died and Went to Heaven.”

This will never happen again.

I won’t rehash it all here, as breathless accounts can be found everywhere from gossip sites to the freaking NY Times. But I just want to stop and note how Tay-Tay is living the most perfect existence of anyone, anywhere on the planet. She has it all: perfect looks, rabid fan-base, rising power, amazing friends, and a string of lovers the envy of every straight female serial monogamist around. And it isn’t a case of “being born on third base and thinking you hit a triple” — her stellar rise truly has Taylor Swift to thank more than any parent, agent, manager or producer.

She’s the one person who does everything perfectly and better than you, and has never experienced a single set-back or failure in her life. She’s the girl who shows up every day looking amazing when you’re happy if your hair is going in fewer than six different directions. That girl in high school is always hated on by the other girls, and always unfairly.* (UPDATE: see below) Swift has no reason to apologize for winning life, nor does she have to apologize for making a diss track about Perry after the latter stole Swift’s backup dancers.

That said… as I’ve talked about before, there is almost no way a white person can talk about race without automatically losing. Look at the show White People, which was designed purely to paint all whites as racists. if you’re on that show, and you are doing anything but reciting politically correct pablum straight out of a college diversity seminar, you are automatically either the schmuck or the villain. Or both.

Part of it is due to political correctness, yes, but it is mostly because we really fail to empathize with people of color because we never experience anything like what they have to go through. No, Nicki Minaj was not snubbed because the VMAs are run by the Klan. But there really is a passive indifference to POCs among whites, even white liberals, that causes this sort of thing, or a fraud like Iggy Azalea being held up as the world’s best female hip-hop artist. And that’s before you get to the more street-level stuff like parents mouthing all the usual PC talking points before turning around and sending their little Ethan to a lily-white private school because their local public school has too many… well, it just hosts kids who are too… oh, let’s change the subject!

Anyway. Yes, I’m still one of those male Swifties everyone gives the side-eye to. I know she meant well, even if she was clueless about it. And I can’t judge. After all, in social situations, I pretty much never have a clue no matter how many clue phones are ringing with urgent clue messages from Clue Headquarters.

And as for actual racial outrages go, the case of Sandra Bland has far more consequence than the friggin’ VMAs from a network slowly fading to irrelevance. A racist white cop decided to get tough with a black woman who did not show him the proper fear. She was thrown in jail after a violent (on the part of the officer) arrest and wound up dead in custody 2 days later. If a white man had been treated like Bland for sticking up for his rights the way she did, FoxNews would have been raging 24/7 like they did for Clive Bundy. Swift committed a gaffe. Officer Brian Encinia committed assault, racial intimidation, and possibly far, far worse if the suspicious “suicide” story doesn’t hold up. Maybe Nicki and Katy’s fans need to talk about THAT a bit more.

* Update: after posting this, I saw a Salon writer proved me exactly right. Money quote: “She’s like the prom queen who drives the brand-new Mercedes to school, whose parents buy everyone cookies: If you defy her you are some kind of envious grouch or killjoy.” Damn, dude! You shouldn’t automatically hate the prom queen just because she would never date you…

Update 2: A ceasefire has been declared, the war is over, conscription into the armies of Team Taylor and Team Nicki is halted and service is once again voluntary, peace has settled across the land and our long national nightmare is over

When is it right to bash America?

Speaking of nationalism, I resent senseless bashing of America for real or imagined flaws that are neither unique or even representative of America.

This got me in trouble on Twitter a while back, when a popular writer took exception to my criticism of her unjustified America-bashing, leading to the usual sort of Twitter pile-on that happens when a plebe dares speak up to an internet celebrity.

But it’s true. Declaring that America is some special, out-of-the-ordinary villain when it comes to women’s rights, say, is utterly ridiculous when even a glance at international headlines immediately proves that false. Or that we are out of the ordinary when it comes to racism. Or religion (one side likes to complain that America is too godless; the other side likes to smirk about how we’re not godless enough).

What it usually boils down to is, a lot of people find smug superiority in judging this country and finding it wanting of their own inflated egos. How fallen and disgraceful this nation of sinners is for their <racism|sexism|violent movies|fast food|etc etc>. Look at how pure and good I am, and how I must suffer the presence of these American sinners as penance for my being so incorruptible!

What I want to hear about are problems that are specific to this country. For instance, our militarism and our overseas misadventures. There hasn’t been a president with a good military policy for a long while now — either Reagan or JFK was our last, depending on your views — and while the Cheney politburo were the worst of the worst, Obama fails to impress either. Why does the American military have to consider intervening at every global hotspot? Who does it benefit? Do we really think we can solve Islamic extremism with drones?

That is a uniquely American problem. Nobody has anything even close to American military power — and the eagerness to use it. China and Russia have little power beyond their own borders. The various Islamic states and entities are limited to regional wars, beyond the occasional terrorist atrocity. Even the UK is generally just America’s junior partner in our blunders — the Tony Blair Doctine, let’s call it. So any discussion of a country meddling too much in other countries’ affairs must begin and end with America.

But for random America-bashing with no proof that we really are doing X so much worse than anyone else? Pffft. Get out of here with that. Us ‘Muricans can’t hear your commie words over the sounds of our gunfire and our V8 engines and our freedom.

Which reminds me of one of my favorite subreddits. It expresses a fondness for our country while simultaneously recognizing how ridiculous it can be. It’s also why the character Ron Swanson was so appealing even to us liberals.

So yes, criticize this country when it deserves it with gusto. But kneejerk anti-Americanism needs to be retired along with the Boomers who introduced it. Because really, if we are so fallen… what other country would you prefer to move to?

Why I back Bernie

First of all, no, it’s not because I think he has a prayer of winning — words which immediately prove that I am not a paid campaign blogger. So don’t even look at me like that. And everyone knows all but one pre-selected candidate on each side are there just for show, for a potemkin primary to distract the plebes from the smoke-filled rooms where the real primaries were decided years ago.

That said, every show of support for someone other than Clinton on our side, or Bush on theirs, is a slap in the face of the Establishment, making the backing of an insurgent an easy choice. And fortunately, our insurgent candidate isn’t a badly-coiffed racist blowhard who decided to insult America’s POW veterans in a fit of pique. (Ok, our guy may also be guilty of being badly-coiffed, but still.)

Say what you will about Bernie, but there is little calculated in his speeches or his interviews. He has no problem taking anti-Establishment positions that would send big donors running for the Hills, so to speak, if they weren’t already there. He sees the problems of our landed aristocracy, our plutocracy, and makes no bones to wanting to fight it.

This is as opposed to Clinton, who doesn’t take a position, who doesn’t utter a single word that hasn’t been carefully scrutinized, focus grouped, triangulated. She does not permit anything but the most softball of interviews, with all questions agreed upon in advance. Hardly any Democrat believes she has a single principle beyond power for herself and for her family, with the only longterm goal beyond that involving setting up Chelsea for her eventual Senate seat and presidential candidacy. (Thus mirroring Jeb’s commitment to his own clan, and their future plans for George P. Bush.) There is not one shred of integrity she would not sell out — she would turn on even longtime ally Planned Parenthood in a second if it would boost her in the general election; and if their current scandal gets any worse, that is a real possibility.

But Bernie is singing from the same hymnal as he always has, from his decades in the wilderness of the Senate, largely relegated to the crazy-uncle role by his own allies. He won’t even back down from his one major heresy — gun rights — when he has every reason to fudge and triangulate on an issue that will be exploited to the full by the Clinton machine.

In addition, like me, he believes more in socialism than in social justice. As Cesar Chavez knew, one of the best ways to help disadvantaged minority populations is to help them financially. I realize that this isn’t as fun and feel-good as tweeting hashtags, but on the other hand, the $15 minimum wage campaign will offer far more real help* to minority and single-mother families than a thousand #BlackLivesMatter rallies. This does not mean social issues like police brutality can be ignored. But it does mean that practical solutions mean far more than loudly declaring to the media just how multi-culti you are, as white college liberals are wont to do.

Principled. Socialist. Honest. Cranky and ill-tempered. How could I not support the guy?

As the informercial guy reminds us: But wait! There’s more!

For conservatives hate him almost as much as Clintonites do! Here is a recent piece at National Review, repeatedly and with relish calling Sanders — and I am not making this up — a “national socialist.” The logic basically goes that Sanders is distrustful of globalism and the various free-trade agreements; therefore, Sanders is racist against our trading partners in China and Latin America; therefore, Sieg Heil! QED. Writer Kevin Williamson, even as he admits that Sanders is the son of Holocaust survivors, finds some random Bernie fan who also likes today’s German (!) welfare state… and, really, what more proof do you need?

If conservatives are comparing Sanders to Hitler already, it shows what a grave threat they regard his politics.

Not his chances, though. We know how it goes. Sanders will have all the luck of both Rand Paul and Rand’s old man. But Williamson rightly recognizes Sanders’ economic populism as a serious threat to conservative and Clintonian (but I repeat myself) ideals of aristocracy and plutocracy; of a tiny, landed, hereditary elite further and forever strengthening its stranglehold on American wealth and culture, unencumbered by estate taxes that might interfere with said hereditary lineage.

In 2017, we’ll have another pro-0.1% asshole with another all-too-familiar last name as president. No question. But someday, perhaps, Sanders’ politics will catch fire. Perhaps people will get sick and tired of the status quo. Perhaps ex-OWS’ers and Tea Partiers and Sanders voters and Paulites will finally realize they have far more in common than any side cares to admit, and throw in together. That is what the donor class fears more than anything else — a populist base able to get over the illusory red vs. blue divide, and unite. How wonderful it would be!

*actually, expanding the Earned Income Tax Credit (EITC) would be more effective in reducing poverty than raising the minimum wage. But since both parties are equally opposed to this, I have to settle for raising the minimum wage as the more realistic option.

After this latest mass murder perpetuated by a typical angry male loner in his 20s, the Islamophobes are out in force. ISIS in action, they say. His first name is Mohammad; what did you expect? We coddle Muslims too much. This is all the fault of liberals and Obama. If only we would stand up to the Muslims. All of this would then just go away.

But the ultimate truth remains the same: there is very little, if any, difference between murderers motivated by white power and MRA thoughts, and murderers motivated by Islamic jihad. They all come from the same twisted evil that also, in unusual cases, creates completely non-ideological murderous animals like newly-convicted James Holmes. With men like this, the evil always comes first. The rationalizations come later.

Here are the following common characteristics between a white power/MRA-type like Roof or Eliot Rodgers, and a Muslim jihadist/ISIS terrorist like Abdulazeez. 1) Male in his young 20s. 2) Isolated, alone, undateable. 3) Complete frustration at #2, choosing to blame woman/society/infidels/etc. for his isolation and inability to obtain sex, rather than himself. 4) Radicalized by online communities, subreddits, or Twitter. 5) Voluntarily and knowingly giving himself up to evil long before the crime. (I think this is more important than the media admits — almost all of these mass murderers make a conscious decision to devote himself to evil long before the actual crime, so these were not random rage killings.)

The specifics of the murders put out by these sort of worthless degenerates will differ, of course, as will their purported motivations… but the kernel of the crime is universal among these young men. If Abdulazeez had been white, he would have aligned himself with neo-Nazis and/or MRAs before murdering; similarly, if Roof had been born into a Muslim family, he would have sworn allegiance to ISIS before committing murders against Christians very similar to the ones he actually did.

The hate comes first; the rationalizations come later.

This is not to say that the existing infrastructures of hate do not deserve blame. As noted before, the twin hate-ideologies of the white supremacists/MRAs, and Sunni jihadis, are otherwise unrivaled when it comes to servicing and encouraging hate in their young, male, sexless constituencies. Nothing else on the planet comes close to the evil of these two movements.

Young male criminals in other societies, untouched by neo-Naziism or Sunni extremism, usually wind up as just your typical street criminal, which is oddly comforting. Say what you will about your local mugger, but at least he isn’t motivated purely to commit evil for the sake of evil. At least he has an understandable motive — robbery — as opposed to committing evil literally just to make the world a worse place for those in his community. Of course, there do exist rare non-ideological psychos like Holmes and Adam Lanza, who are no less consumed by the worst evil of the human soul; however, without the malign direction provided to neo-Nazis, MRAs, and Islamist jihadis, most such men do not go to such extremes when common street crime is so much more accessible.

If you want an answer to why four honorable servicemembers of the Corps had to die at the hands of this Muslim, you have to look into the same dark places that foment the Dylann Roofs and Timothy McVeighs that infest our country like cockroaches. Because to the grieving widows, the specifics are ultimately meaningless. The hate came first. The rationalizations came later. Evil is evil.

Last year, I had a 52 year old man come in with shortness of breath.

This is a fairly common complaint in the urgent care. Asthma attacks help keep the lights on at a clinic like this (our accountant asks parents to yes, please, keep smoking around your kids), and the patient did have a history of mild asthma. However, his usual inhaler pump isn’t working. His shortness of breath (yes, we abbreviate it “SOB”; giggle away) has been getting progressively worse over the past 2 weeks or so. It is much worse at night time or when he has to take the stairs. He also noted a weight gain of “at least” 25 pounds over the past month. He denied any pains, fevers, urinary or stool problems, head or sinus congestion, or recent travel. He maybe had a slight cough. He’s also had swelling of the ankles develop. His chronic medical issues included mild asthma and mild hypertension. His blood pressure today was 138/90. His oxygen level was 94%.

BY THIS POINT I WAS 100% SURE OF THE DIAGNOSIS. WHAT IS YOUR GUESS?
